Application for Degree or Certificate of Achievement
Once you have determined your eligibility for a degree or certificate by consulting an academic counselor, use this form to apply for your degree or certificate.

Substitution/Waiver Petition of Graduation Requirements
Request a substitution or waiver for one or more classes required for graduation if you feel you have satisfied the requirements in another class or by other means. Use only for the Associate Degree (AA/AS), Certificate of Achievement, Skills Comp Award, and/or Department Award. Submit the completed form to diplomas@sbcc.edu.
Note: This form is not used for transcript evaluation or prerequisite clearance. See the Transcript Evaluation Office for more information.

Nonresident Tuition Exemption 

Academic Renewal Petition (PDF) - Currently enrolled students must have an academic counselor sign the petition. Contact your counselor for assistance. The online petition form through Etrieve is currently unavailable. Any petitions submitted using that format will be returned unprocessed.

Late Add Petition Online Form  Please read these instructions for submitting and checking on the status of your online form. The deadline for full session length courses for our 2022 Summer Session II was Friday, July 8th at 11:59 pm (25% length of the class for classes less than full session length).  Late Add Petitions submitted after the deadline will be denied.

NOTE: adding a preferred first name to your records will display on your campus ID card and on course rosters.

Skills Competency Award Application (PDF)
Do not use this form to apply for Associate Degrees or Certificates of Achievement. Skills Competency Awards do not appear on SBCC transcripts and are not included in Commencement.
